Transition

Transition is the word we use for moving between something familiar to something different. When you move from one school to a different school, this is a transition.

Transitions can be small, like moving between different places during the day. Or they can be bigger, like moving from a children's doctor that you trust, to having appointments with a new doctor for adults when you turn 18.

There are lots of things that you, your parent/carers and local support services can do to make these sorts of changes more comfortable, and a bit less scary.
